Role Overview:

  • Assess and enhance health needs for Looked After Children.
  • Implement and evaluate healthcare plans.
  • Work autonomously and collaboratively within a multi-disciplinary team.

Requirements:

  • Permanent position based at Woodlands House, Cardiff.
  • Desirable: Ability to speak Welsh; both Welsh and English speakers welcome.
